All First Aid incidents should be reported using this online form.
If you have more than one incident taking place, you can open a new tab in the browser.
You should keep the brower open until a parent/gaurdian is available to confirm the contents of the incident, then you can submit. All completed forms are saved in Wix.

Injury Classification

Abbreviated Injury Scale (AIS) AIS 1 Minor -Small injuries like bruises & scrapes

AIS 2 Moderate - Sprain, Deep Cut, Concusion (e.g. may require follow up treatmeant)

AIS 3 Serious - Ambulance/Immediate Hospital Treatment

Note: if the parent is not available to sign, and the AIS Injury scale is AIS 2 or above we will email the parent the accident form and ask them to acknowledge it.
